Outreach and Community Engagement

Our perinatal case managers meet with patients one on one and offer sexual and reproductive education. This also allows them to identify and address any resource needs, especially those that may affect a current pregnancy. This includes linking patients to services such as prenatal care, WIC and the Nurse Family Partnership. Our perinatal case managers connect with their clients at least once per month and follow them to delivery. Through their efforts, our perinatal case managers have strengthened outreach efforts related to PrEP and HIV, along with relationships with local providers and community-based agencies.
